Should I include a card with a wedding gift?

It's a blender in a box. Should I include a wedding card (from Walgreen's or something) with it and tape the card to the outside of the wrapped box? Or should I just give the wrapped gift? Do people usually write To: and From: on wedding gifts like on Christmas gifts?
